Description¶
An NHS-approved tablet-delivered falls-prevention programme co-developed with older adults and falls-prevention experts, combining progressive strength-and-balance exercises with eCoach guidance, safety information, reminders, progress tracking, and health-literacy games for older adults.
Components¶
- Keep-On-Keep-Up (KOKU) digital falls prevention programme: KOKU is an NHS-approved, tablet-delivered digital programme offering "a progressive strength and balance programme with animated characters (eCoaches) providing instructions, guidance, safety information, automated reminders and in-app progress tracking", alongside health literacy games on nutrition, hydration and falls awareness. It was co-developed with older adults and falls prevention experts using behaviour change techniques and MRC complex-intervention guidance. In the trial it was used 20–30 minutes, three times a week for 12 weeks at home. (French Chloe et al. (2026))
Claims¶
- The KOKU digital programme improves balance function at 12 weeks compared with standard care in community-dwelling older adults, with the effect increasing over time [+M]
- The 12-week KOKU programme is dominant over usual care in cost-effectiveness, with a significant QALY gain and non-significant cost savings [+M]
- KOKU improves lower-limb function, concerns about falling and health-related quality of life, but not mood, physical activity, fatigue or fall rate [+M]
- Older adults rated the KOKU digital programme with excellent usability and high acceptability [+M]
- Qualitative findings: ease of use supports engagement and confidence, but challenge must be tailored to ability and content must be culturally relatable [+M]
