Align CPD activities, programs, and systems so they function in collaboration to support physician SDL¶

Description¶
The review recommends that, for the best results in physician engagement in SDL in CPD and measurable changes to clinical practice, "the different activities, programs, and systems within CPD need to be aligned with each other and function in collaboration." It further recommends that future studies address knowledge gaps about both physician engagement and SDL developer implementation, since implementation depends on factors affecting the entire spectrum from development and delivery through receipt and enactment.
Design Implications¶
Context¶
Requirements¶
- Attention to barriers and facilitators at both individual and external levels, for physicians and SDL developers alike
Constraints¶
- The recommendation rests on a scoping review of 17 Canadian articles published 2005–2015, limited to English-language scholarly literature
Target Learners¶
- physicians in continuing professional development
- CPD program developers and deliverers
Target Learning Goals¶
- sustained engagement in self-directed learning and measurable changes to clinical practice
Claims¶
- Barriers and facilitators to SDL in CPD occur at five levels: individual, program, technological, environmental, and workplace/organizational [+W]
- Most SDL-in-CPD behavior-change factors fall in four TDF domains: environmental context and resources, social influences, beliefs about consequences, and behavioral regulation [+W]
- Time constraints are the most frequently cited physician barrier to SDL engagement, within the environmental context and resources domain [+W]
- Resource mismatch, missing incentives, and discontinuous post-training support constrain the sustainability of training transfer [+W]
- Physicians engage in SDL programs more when they perceive the content as useful and relevant to their clinical practice [+W]
