Multi-component communication intervention (DTT + JASP+EMT+SGD + caregiver training) for preverbal preschoolers with autism¶

Description¶
A 36-session adaptive intervention delivered over 4 months combining three components: 'therapist-implemented DTT with the child to teach foundational skills', therapist-implemented JASP+EMT+SGD during play and routines, and caregiver training to implement JASP+EMT+SGD. Sessions occurred two clinic days and one home day per week (45–60 min), with three caregiver workshops. The intervention was adaptive: caregiver practice time increased with caregiver skill fluency and child DTT decreased as foundational skills were mastered. All families also received an iPad with Proloquo2Go and caregiver SGD programming training.
Design Implications¶
Context¶
Requirements¶
- Children must be preverbal or at first-words stage (fewer than 20 different spontaneous words in a 20-min language sample) with a confirmed autism diagnosis
- Master's-level therapists meeting fidelity criteria for all components; the same therapist implements all components for a family
Constraints¶
- Components were implemented at lower doses than previous studies of DTT and clinician-delivered JASP-EMT+SGD, which the authors state may have impacted effectiveness
Target Learners¶
- preschool children with autism (ages 3–5) who are preverbal or minimally verbal
Learning Goals¶
- joint attention
- social communication
- spoken language
- foundational skills (imitation, receptive language, SGD use)
Claims¶
- Multicomponent Intervention Improves Joint Attention Post [+M]
- Preschoolers with autism given a multi-component communication intervention used more social communicative utterances with their caregivers than comparison children 4 months after it ended, a small difference that was not significant immediately after intervention [+W]
- Caregivers Acquire Maintain Strategies [+M]
