Transfer-first governance framework bounding non-neurosurgeon temporising cranial intervention¶
Design · All designs

Description¶
The article proposes a conceptual framework (Table 4) mapping each included pathology to a time-criticality level, an intended role for the non-neurosurgeon, minimal prerequisites, and a transfer principle. EDH with deterioration maps to time-critical temporising decompression only when transfer is infeasible; acute SDH is a rare "in extremis" exception; intraparenchymal hemorrhage is primarily educational exposure. The framework's core is that "NEUROSAVE is best understood as a structured educational intervention that prepares non-neurosurgeons to execute strictly bounded temporising steps within an explicit transfer-first system, not to expand the scope of independent operative practice."
Design Implications¶
Context¶
Requirements¶
- Predefined indications, neurosurgical consultation, and audit; checklist, defined instrumentation, supervision plan
Constraints¶
- Any real-world application of ICH exposure should remain highly restricted and protocol-defined; implementation should remain protocol-bound, transfer-first, and embedded within defined consultation and audit processes
Target Learners¶
- general and pediatric surgeons in regional hospitals without on-site neurosurgical coverage
Learning Goals¶
- executing strictly bounded temporising cranial steps as a bridge to definitive specialist care
