Increase vaginal twin birth rates through provider-focused interventions: staff-wide training or specialist teams¶

Description¶
The article demonstrates that two provider-focused strategies—quarterly simulation-based training for all labor ward staff, and dedicated specialist teams skilled in twin and breech vaginal birth—can substantially raise vaginal twin birth rates without worsening outcomes. As the authors state, "a rapid increase in the vaginal birth rate of twins is possible via either staff‐wide training or specialist teams without adversely affecting maternal or neonatal outcomes." The principle applies in settings with high background cesarean section rates where few clinicians have current vaginal twin birth experience.
Design Implications¶
Context¶
Requirements¶
- Providers must receive training in twin delivery, breech delivery, shoulder dystocia, operative vaginal delivery, and other obstetric emergencies, typically via lecture plus hands-on practice on an obstetric mannequin
Constraints¶
- Results may not be replicable in hospitals where staff are unsupportive of vaginal twin birth or where specialists cannot form supportive teams; the study examined only twin pregnancies with the first baby in vertex position
Target Learners¶
- obstetricians
- midwives
- resident physicians
Learning Goals¶
- vaginal twin birth management
- breech delivery skills
- management of obstetric emergencies
Claims¶
- Vaginal Twin Birth Rate Increased After Training Interventions [+M]
- No Significant Change Adverse Outcomes Twin Birth Training [+M]
